New Mexico Department of Health Developmental Disabilities Supports Division Health Assessment Tool Project Overview • Introduction • The Developmental Disabilities Supports Division (DDSD) requests Certification for Planning Phase for the project to implement an electronic health assessment tool. • Overview • Replace paper-based tool with electronic version. • Improve provider accessibility, quality and tracking abilities. • Expand tracking for client services. • Improve and expand reporting and data analysis capabilities. • Compliance with Court ordered activities and HIPAA. • Procurement via RFP process
