Equipment: - Clean urinal. - Toilet tissue. - Equipment for a specimen if required.
Preparation: Assist the client to an appropriate position. 1. Females confined to bed may prefer a semi-flower's position.
Performance: 1. Explain to the client what you are going to do, why it is necessary, and how she can cooperate. 2. Wash hands and observe appropriate infection control procedures. 3. Provide for client privacy.
4. Assist the client with using the urinal. * Offer the urinal so that the client can position it independently. Or * Place the urinal between the client's legs with the handle highest so that urine will flow into it. * Leave the signal cord within reach of the person. The client can then call for assistance if required. *Leave for 2 to 3 minutes or until the client signals. Or * Remain if the client needs support to stand at the bedside or other assistance.
5. Assist the client with removing the urinal as needed. * If wet, wipe the area around the urethral orifice with a tissue. * Make sure the perineum is dry. * Offer water, soap, and a towel to wash and dry hands. * Change the drawsheet if it is wet.
6. Attend to the urine as required. * Measure the urine if the client is on monitored intake and output, and provide a specimen if required. * Empty and rinse out the urinal, and return it to bedside unit. * Wash your hands. 7. Document finding in the client record using forms, record the amount of urine, if it measured, and all assessment data (e.g., cloudy urine, reddened perineum).
