  2. If you do not have an account, click ‘Request an account’ and an administrator will grant you access after you fill out the registration form.

  3. At the Login screen, enter your Username and password in the text boxes on the left of the screen, then click the ‘Login’ button.

  4. Click for plant sample submission Upon login, you will see a list of samples, if you have submitted any in the last 30 days, and their corresponding information. To submit a plant sample, click ‘Plant Submission Form’.

  5. For existing clients For prospective clients Select client via their Bartlett ID number or last name (existing clients ONLY). For prospective clients enter their information under Option 3.

  6. Scroll Down Fill in the submission form (Red fields are required) with sample information. Be sure to indicate if digital images are being submitted by selecting the correct sample type.

  7. WRITE SOMETHING! Site history, suspect anything, girdling roots, root rx, construction Please include as much information as possible.

  8. Up to 5 digital images can be submitted Attach digital images (if there are any) by clicking ‘Browse’. This opens a window allowing you to select image(s) to attach to the sample. When complete, click ‘Submit Form’ button.

  9. Image can be previewed by clicking ‘Preview’

  10. Click This Link Upon successful submission, click the PDF link to take you to the submission information sheet. If a physical sample is submitted, this MUST be printed.

  11. This is the sample information sheet that MUST be included with a physical sample. After printing, close out the Adobe Acrobat window to return to the submission system.

  12. a b Click ‘Home’ and the arrival/diagnostic status of your sample is displayed (a). You may also print a submission form from this page for a sample if you forgot to previously (b).

  13. Click here to go directly to sample This email is for your info only! If a client copy is desired, follow the link to build a copy.

  14. Diagnostic Information To view a diagnosis, click the sample number of a diagnosed sample. At the bottom of the page, you will see the diagnosis and recommendation section. You may also generate a PDF document suitable for printing or emailing to the client.

  15. Check off the images to include on report If digital images were submitted with the physical sample, select those you would like to include in the diagnostic report. Up to 3 images can be included in the report.

  16. Click to attach a tech report The sample submission system also contains the ability to e-mail diagnostic reports directly to the client. If this option is chosen, a Bartlett Tech Report can also be attached by clicking on the appropriate button.

  17. This is what the client PDF report looks like. This can be attached to an e-mail to the address entered on the web page. After reviewing or printing, close the Adobe Acrobat window to return to the system.

  18. By clicking the ‘Submitted by’ link, you can view an archive of previously submitted samples and information about them. Click the sample number to retrieve the diagnosis

  19. By clicking ‘Search’, this allows you to search the database of samples to retrieve previously diagnosed samples. Search by keyword, sample number, or client name .
