  4. Major Elements Three Focus Areas: 1. Goals of initiative • Prepared for and contributed to national accreditation program • Promote application of QI methods 2. Quality improvement • Adoption, reach and impact 3. Collaborative approach • National and local collaboratives

  6. Annual Survey • Purpose: • To assess goals of initiative & QI capacity, adoption and reach in MLC-3 states • Administered to: • All health departments in MLC-3 states • Timeline: • Administered annually • Scheduled for Nov-Dec, 2008

  7. Case Studies • Purpose: • To assess efforts of Mini-Collaboratives in four of ten target areas • Participating Sites: • 12 case studies in four target areas • Timeline: • Scheduled to begin in winter, 2009

  8. Collaborative Survey • Purpose: • To assess MLC-3 participant perceptions about the success of the Collaborative • Administered to: • Representatives from MLC-3 states • Partner agencies • Timeline: • Year three

  9. Key Informant Interviews • Purpose: • To assess perceptions about the National and Local Collaboratives • Administered to: • Representatives from MLC-3 states • Partner agencies, consultants, funders • Timeline: • National: Annually, in spring • Local: Coincide with case studies

  10. Process Tools • Feedback Forms and Monitoring Tools • Under development • To explore: • Implementation of efforts • Information exchange • Organization of the Mini-Collaboratives • Resources and supports needed
