Description of Transfer Process • Identify children in 3 phases of transfer ----------------------- • Set dates • Collect information • Hold transfer review meeting • Complete the Summary document • Submit to LA ------------------------ • Write EHC plan • Publish proposed EHC Plan • Offer personal budget • Mediate any outstanding difficulties
