Key Problem: Acute Ambulatory Services (2007) needed to be responsive to meet the needs of SH and the service users: • improve access to services • divert and substitute care to relieve hospital demand • adopt universal approaches maximising efficiency, workforce flexibility and transfer of resources • provide models of care that reflected the needs of the client and SH
Aims of this project: Phase 1 • To improve governance in patient care • Retain staff through service redesign • Implement a career structure • Implement new models of care Phase 2 • Reduce reliance on brokerage by converting dollars spent to EFT
Key Changes Implemented: Phase 1 Implementation of a comprehensive career structure • Introduction of Grade 2 RN nurses into HITH • Introduction of a Clinical Educator • Introduction of CNS’s and CNC into the service • Introduction of a Grade 4A • Two new bundles of care: Cellulitis and Anti-coagulation Phase 2 Cost benefit analysis of brokerage to EFT conducted Project plan to be developed September 2009 .
Outcomes so far: Patient Care The hospital’s utilising the new HITH medical model. This has had significant reductions in LOS for cellulitis, patients
