  3. #1) Remove Scope-of-Practice Barriers - RNs in the community • In the Blueprint for Health model RNs working to the full extent of their education might: • Actively help manage chronic conditions through patient education (i.e nursing visit for group instructions) • Assist patient with goal setting for self-management, • Assure screening and preventative measures such as lab tests and immunization • Lead the team’s use of electronic health records to track outcomes

  4. #1) Remove Scope-of-Practice Barriers - RNs examples • SCHOOLS NURSES - Studies show that the availability of a school nurse correlates with better school attendance, which in turn predicts better academic performance. • HOSPITAL NURSES -Daily rounds at Seton Northwest Hospital in Austin, Texas, now include a nurse along with other members of the interdisciplinary care team. Seton was 1 of 10 hospitals selected to participate in Transforming Care at the Bedside (TCAB). Patient care has improved and nurses enjoy better communication with physicians since the TCAB team determined that nurses and other members of the care team should accompany physicians during their rounds.
