This is supporting work of the NW Housing LIN Leadership Set • Do you have examples of transformative health, care and/or support services that demonstrate greater cost effectiveness, reduce demand, improve quality, deliver better outcomes, showcase innovative new models of care at home or housing development etc for inclusion in our report? • If so, please email info@housinglin.org.uk a short description of your services for consideration, by Tuesday, 3 January 2017.
