Lesion Description • The lesion is a widespread area area of erythema with a peripheral zone of white that appears to involve the epithelium on the posterior dorsal area of the tongue.
Patient Dialogue and Responses • Have you noticed a lesion like this before? PT: Yes I have. • If so, does the lesion seem to come and go? PT: Yes, for as long as I’ve noticed it. • Does the lesion appear during highly stressful times? PT: It definitely appears during the highly stressful times in my life. • Are you currently taking any medications? PT: No
Patient Dialogue and Responses • Do you have any allergies? PT: No • Have you been diagnosed with diabetes or Reiter’s Syndrome? PT: No
Diagnosis Differential Diagnosis: Candida, fissured tongue, geographic tongue Provisional Diagnosis: Geographic Tongue (benign migratory glossitis) because the lesion’s appearance is consistent with this diagnosis. The lesion is asymptomatic, does not wipe off, comes and goes with stress, and is an area of erythema surrounded with a white border.
Treatment • If the benign migratory glossitis becomes symptomatic, it can be treated with topical anesthetic, crushed ice, or topical histamine.
