Census • 32 active pregnant women • 8 post delivery • Phases • Phase 1 • Enrollment through post 6 months delivery • Phase 2 • Post 6 months from delivery to 18 months post delivery
Since Institution of program • Over 400 babies delivered drug free (99% of enrolled members delivered drug free) • Added support services, in addition to medical service • Mental health assistance • Court assistance • DHS assistance • Education resources • Employment guidance
Community Support • Marion and Polk County Health Department • Various mental health support services • Providers • Peers/members direct referral to coordinator • Hospitals • Moving Forward • Promote outreach with community support • Police, district attorney • Schools, college campuses
