  7. My Pilot • Identify groups with carer needs Multiple Sclerosis Stroke Palliative & Cancer patient Head & Spinal Injured Mental Health Drug & Alcohol dependence Epileptic Carers Register

  8. My Pilot - Results • 8580patients registered • 402 patients identified • 61 mental health • 118 drug and alcohol problems • 45 palliative care • 40 recent cancer diagnosis

  9. My Pilot - Results • 48 potential Young Carers highlighted within selected categories (5-19 yrs) • 1 Young Person on Carers Register

  10. Potential Young Carers • 566,600 residents in Worcestershire • If results are typical • potential 3,000 young carers in Worcestershire • Currently Young Carers Service supports 238 children and young people

  11. Using the Evidence • School Health Nurses • Highlight on School Health Records • Quietly monitor • Advertise within Schools • Use social media: Twitter, Facebook, school websites

  12. Using the Evidence • Work with GP practices • Highlight vulnerable families on IT system • Set up carers register within practices • GP’s signposting on to school health & young carers
