  5. A Simple Dental Options team member will notify the office by phone or IM within a few minutes with an approval/denial status along with the qualified allowance and payment program options. If the patient is denied, a denial letter will be sent to their home address.

Documents to sign & make a copy for the patient are: Summary of Loan Terms including the Consumer Loan Agreement Promissory Note Authorization/Release of Patient Information Payment Election Form Scan signature pages into document center or place in patient’s chart.

  7. Summary of Loan terms including the consumer loan agreement …this explains the costs, payment break-down and key terms of the loan and the borrower’s obligations and the consequences of the failure to pay

  8. Promissory Note … is the physical evidence of the borrower’s obligation to pay, like an IOU or a Treasury bond certificate.

  9. Authorization/release of Patient information …this allows us to securely give your information to 3rd parties, such as Simple Dental Options.

  10. Payment electionform …this allows the patient to give us the ability to automatically make their payments through savings/checking or credit card.

  11. Fax all signature pages to Simple Dental Options at 866-573-8534 or email if scanned to customersupport@simpledentaloptions.com

  12. Complete treatment and Charge out accordingly. Collect the patient’s down payment as shown on patient’s summary of loan terms and post into Dentrix. Insurance estimates can be applied towards the down payment.

  13. The Simple Dental Team will adjust the patient’s account accordingly and flag the account in Dentrix. The remaining account balance (only the amount financed) will be removed from Dentrix. All statements and collection processes will be handled by Simple Dental for the amount financed only. All monthly payments will be made to Simple Dental Options. The Simple Dental Options team will adjust patient’s accounts accordingly in Dentrix and post payments as they are received.
