IABP flight logistics Dec 2013
Prep for / Retrieve IABP • Remove Critical Care bag from aircraft / leave in hangar. • Take ICU cell (?need) & CPAP cell (in case you get a flight after turning your pt over to receiving unit) • Bring add’l IV infusion pumps + tubing, if available • Pedi bag to stay (belt in forward of IABP pump) • Bring heater with you in winter – assure belted in. • Retrieve pump + bag w supplies from WCMC ED ?? • Secure into aircraft mount/remove console & secure
If picking up patient from WCMC: Plug aircraft in to shore line • Plug IABP into wall of helicopter (adapter next to right side of IABP) • Once helicopter started, turn on 110 v inverter switch – should be illuminated (green)
Patient care @ bedside • Assess your pt, as with any critical care pt • Remember to assess radial/pedal pulses, √ insertion site, √ IAB cath tubing for blood • Determine pt hemodynamics / IABP settings • Place our IABP ECG leads on the patient (be sure to leave the patient's current leads on – the Refer IABP is most likely on ECG trigger) • Place Zoll posterior pacer pad on • Prepare Art line set-up (if not compatible) • Complete ventilator / IV infusion transfer
Travel to aircraft As you walk with patient out to helicopter, someone holds the balloon tubing near the groin and the person pushing the balloon pump holds the tubing near the IABP connection Two sites to exit • First site: Shorter distance • Exit out of the rear door / behind the ED • Take care not to tip Refer IABP / not secured to wheel base! • Second site: Longer distance • Through the ED and out the side door • More room to maneuver so may be site of choice if numerous pumps etc.
To Transfer IABP IABP in aircraft • Load front of stretcher into locking mechanism • Turn on IABP. Open Helium. Program settings to match what pt is on. • Connect ECG cable and Art line cable to IABP • IABP takes about 60 seconds to self test. • Switch helium line • Push START on IABP • Load stretcher in w care
Communication is the key! • Remember to determine where the IABP is located, prior to lifting off. • Sometimes it travels!! • √ Helium Tank > 25% full (spare tanks @ MC office) • You MUST coordinate well in advance if you are requesting that the Receiving nurse meets you with their IABP. • This may not always be possible! Know how to remove our IABP to take to Rec. unit if necessary.
