  3. Open Enrollment Open Enrollment 10/1/2009 through 10/31/2009 All changes and enrollments should be submitted to HR by 11/2/2009 Changes and enrollments will take effect 1/1/2010

  4. Open Enrollment • An Employee Profile will be distributed that is a “snap-shot” of your coverage. Please review and submit changes to Human Resources. • An Enrollment Guide is available that offers a quick overview of plan benefits as well as enrollment/change forms (which also list the bi-weekly cost of insurance). • Enrollment materials will be sent to sites for distribution.

  5. Changes to plan • In-Network In an effort to control costs, our PPO network will change to Aetna Signature Administrators (ASA) for all areas except Wisconsin. Note: New health plan booklets and IDs will be sent to participants in January. • Information about the Aetna Signature Administrators network can be viewed at: www.aetna.com/asa • Out-of-Network Will only be covered at 50% instead of the current 60% for Basic and 70% for Basic Plus plans. Note: There will be no maximum limit to out-of-pocket expenses for these services.

  10. Voluntary Life Insurance • Annual open enrollment each year at $10,000 • Annual open enrollment for Spouse and/or dependent children for $10,000 • One level increase without providing medical information. • Rates have not changed for 2010

MetLife Perks • Will Preparation Service You will receive information once your coverage becomes effective under MetLife • Travel Assistance ID Card Help with travel information, medical assistance while traveling, lose documents, credit cards or luggage while traveling,you are the victim of identity theft and need assistance

  13. Unum – Voluntary Benefits • Coverage for Long Term Disability, Short Term Disability, Accident and Critical Illness are being provided through Unum. These plans are employee paid and offered at group rates, making this coverage more affordable and possibly below the cost of other providers.

  14. Unum – Voluntary Benefits • Long Term Disability theSalvation Army provides a base amount of coverage, but all employees have the option to buy-up from there to cover a larger portion of their income in the event of a covered accident or illness.

  15. Unum – Voluntary Benefits • Short Term Disability income protection insurance can provide benefits if you become disabled and cannot work due to a covered illness or accident. Available monthly benefits range from $400 to $5,000 per month and are paid in addition to any other insurance policy you may have. Your coverage is guaranteed renewable to age 65 as long as premiums are paid when due and premiums will not increase due to age (rates may change on a class basis). Premiums staylevel and you can port thecoverage.

  16. Unum – Voluntary Benefits • Critical Illnesses pays a lump sum of $5,000 - $50,000 if you are diagnosed with any of the following covered illnesses, including heart attack, stroke, major organ transplant, end-stage renal failure, permanent paralysis, coronary artery disease and cancer (internal, skin and carcinoma in situ. The benefit can be used any way you choose, and you don’t have to be disabled or terminally ill to receive benefits. Coverage is available for you, your spouse, and children.

Unum – Voluntary Benefits • Accident Insurance pays for covered accidents while on the road to recovery Unum’s coverage provides a lump sum benefit based on the type of injury (or covered incident) you sustain or the type of treatment you need. You can purchase coverage for you, your spouse, and your children. Coverage is portable and premiums are level.

Unum – Voluntary Benefits • Unum Representatives will be available via phone at (866) 703-0286 during the month of October to enroll employees and/or discuss any of these voluntary plans. • Since these are new plans, waivers will be required from employees who decline to participate in all 3 of these employee paid plans. • Waivers will need to be submitted to HR by 11/2/2009

  19. Pre-tax benefits : Flexible Spending Accounts & Section 125 • Flexible Spending Accounts (FSA) - These accounts allow you to reduce out-of-pocket costs for health services (medical, dental, vision) as well as daycare expenses by using pre-tax dollars. Up to $1,000 in medical costs and up to $5,000 in daycare costs can be designated as a pre-tax deduction each year. This amount is put into a spending account that you can withdraw from to cover related expenses. Applications are needed if you are interested in continuing or beginning this benefit. • Section 125 Benefits (pre-tax deductions) - With increasing costs associated with insurance coverage, we offer a Section 125 Plan that allows your monthly contributions to be on a pre-tax basis. This option is only offered at open enrollment and a Section 125 form is required.
