3. 3 What are we dealing with? What kinds of things go wrong?
These photos are selected to illustrate some general topics for discussion:
Big and small don’t mix
“Dead truck” syndrome
Vehicle access
Housekeeping
Fit-for-purpose
Most hazards are now well managed but mines inspectors sometimes get a feeling of déjà vu (e.g. “dead truck” syndrome)
4. 4 BIG AND SMALL DON’T MIX Let us take a look at some of the kinds of things that inspectors have to investigate as part of their work.
While these may be on a larger scale than many of you will have to deal with in the course of your jobs, the principles of investigation do not vary.

5. 5 What’s happened here? Size, weight and power of mobile machinery used in mining operations present significant hazards to people and equipment in area of operation
In particular, operator has restricted view of surroundings

10. 10 What’s happened here? Too close to edge?
Too steep?
Too fast?
Overloaded?
Poor road design?
Poor road maintenance?
Not fit-for-purpose?
What else?
Wearing a seatbelt usually improves the outcome

13. 13 VEHICLE ACCESS
14. 14 Is it safe? Due to their size and height of platforms involved, large mining equipment and vehicles can be difficult to:
Access
Alight from
Maintain
Clean
