  1. 2010 - Fatality – No. 1 – Signal Peak Energy, LLC – Bull Mountain Mine No. 1 A 57 year of mechanic with 8 years of mining experience was fatally injured at a surface shop of an underground coal mine. He was repairing this 1-ton truck used as a mantrip for underground transportation. He had the front of the vehicle support by jack stands while his co-worker went inside the cab to perform a task requested by the mechanic. His co-worker depressed the clutch and started the engine BUT FAILED TO PUT THE TRANSMISSION IN NEUTRAL, his foot slipped off the clutch and the vehicle jerked forward knocking the front of the vehicle off the jack stands. • Block wheels, set parking brake and have transmission in neutral. • Get out from under the vehicle until all precautions are taken prior to starting the engine!

  2. 2010 Fatality No. 2 – Bledsoe Coal Corp. – Abner Branch Rider in Kentucky. A 29 year old miner operator was fatally injured when a rib roll approx. 70” high by 63” long and 103” wide fell. The rib fell and struck the miner operator while he was cleaning up a previously bolted crosscut. Geological conditions changed! Always be alert to changes in roof or ribs

Fatal No. 32 – Machinery – Surface of Underground – Illinois – M-Class Mining LLC A 61-year old contract iron worker/mine fireboss with 20 years of mining experience was fatally injured while installing pre-fabricated metal stairs on the side of a fan housing. The rigging (clamps) that held the stairs in place opened unexpectedly as the stairs were being hoisted allowing the top end to fall. This caused the bottom end of the Stringer to pivot up and swing out thus pushing the victim backward and pinned him against a nearby manlift. PINCH POINTS STAY OUT OF • Discuss work procedures and identify all hazards associated with the task, THEN • Communicate, Communicate, Communicate your actions before doing anything, THEN • Ensure that all personnel stay clear of hoisted loads as the material is started to be moved.

Fatal No. 33 – Machinery – Underground – West Virginia – ICG Beckley LLC A 28-year old continuous miner operator with 5 years of experience was fatally injured with he was crushed between the boom of the miner and the coal rib. RED ZONES STAY OUT OF • Ensure personnel are positioned beyond the turning radius and away from the • conveyor boom turning radius before starting or moving the equipment. • Shut off miner when it is necessary to be alongside of the machine for any reason. • If necessary to be alongside of the machine with pump motors on, activate the • tram breaker, get out of the RED ZONE, test it, and then do what is necessary
