NUHS Family Medicine Residency Program.

NUHS Family Medicine Residency Program NUHS, in collaboration with St Luke’s Hospital, Raffles Medical Group and Frontier Healthcare, is launching an innovative Family Medicine Residency Program that offers our trainees a diverse experience in an acute tertiary academic hospital, community hospital and private GP settings. This program exposes the FM Resident to a public-private mix that extends the breadth of primary care exposure to firsthand experience of private GP practice in graduated blocks of learning, a FMC focused on continuity care and chronic disease management, specialist ambulatory care experience in NUHS, and rotating hospital postings. Leveraging on the strengths of each of the participating institutions, trainees will experience breadth and depth in their residency learning and a continuum of care in different settings. The Resident will also develop competency in assessing and meeting the healthcare needs of our citizens from young to old -- in acute care, continuing care, home care and end-of-life care. STRENGTHS OF OUR PROGRAM: • Continuity Clinic at our Family Medicine Centre where they will receive mentorshipby a panel of Family Medicine Physician Faculty. • Inpatient rotations at NUH • Structured training in Adult and Paediatrics Medicinethrough inpatient rotation and NUHS Specialist Ambulatory Carerotation. • Faculty has vast experience in preparing trainees for the MMed Family Medicine Exams being involved in developing and organizing the exams since its inception • A sample trainee’s rotation is as follows:-

His passion lies in the integration of care across the diverse settings that patients will traverse in the course of their care. Sc (Hons), GDFM (S'pore), GDOM (S'pore), DFD, DME (Aviation), MCFP (S), MMED (Fam Med) (S) Dr Chng is the Deputy Medical Director of the Raffles Medical Clinics and currently works as a Family Physician in Raffles Executive Medical Centre at Raffles Hospital. He is a tutor in the Graduate Diploma of Family Medicine course and also serves as an assistant supervisor for the Master of Medicine in Family Medicine (Programme B) training. He also teaches in undergraduate family medicine programmes in the Yong Loo Lin School of Medicine. He is responsible for charting the corporate direction and business strategies of his group practice. Apart from his corporate management responsibilities, he is also a practising family physician. Dr Tham Tat Yean Core Faculty Dr Tham is a Board Member of Agency for Integrated Care (AIC), a subsidiary of MOH Holdings. He is a Council Member of the College of Family Physicians Singapore (CFPS). He is a former Council Member of the Singapore Medical Association (SMA) for several years and has served as its Honorary Secretary. Dr Tham has served in various Ministry of Health committees including the Clinical Advisory Committee for Chronic Disease Management, Advisory Committee for National Diabetes Strategy and Clinical Practice Guidelines Subgroup on Prostate Cancer Screening. He is presently a member of the following: National Transplant Ethics Panel of Independent Medical Practitioners, Singapore Medical Council’s Complaints Panel and Screening Tests Review Committee (Academy of Medicine). He also co-chairs the GP Advisory Panel (AIC). Dr Tham is an Adjunct Assistant Professor in the Department of Medicine, Yong Loo Lin School of Medicine where he teaches Family Medicine to undergraduate medical students. He is a Tutor in the Graduate Diploma of Family Medicine programme, a postgraduate Family Medicine course conducted by NUS.
