ARRA ACT American Recovery & Reinvestment Act • $27.5 Billion (Nationwide Highway Infrastructure) • $3 Billion – Total combined ARRA funds to Connecticut • $455 Million CTDOT • $302 million highway projects: • $202 million road and bridge construction • $90.6 million sub-allocated to MPO’s (regional projects) • $9 million enhancement projects • $153 million Transit + Rail

REPORTING FORMS • Required Reporting Forms: • 1201 – Certification Maintenance of Effort • 1511 – Certification • 1512 – Recipient Report • 1585 – Monthly Recipient Project Status Report* • 1586 – Initial Project Information • 1587 – Monthly Summary Employment Report • 1588 – Periodic Grouped Project Report • 1589 – Monthly Employment Report* • 1607 – Certification • 1608 – Section Status and Progress of NEPA Projects • Table 1 – T & I Aggregate Data by Formula Program • Table 2 – T & I Aggregate Data, Project Specific • Table 3 – T & I Coordinator Information • Table 4 – T & I MPO Information • Miscellaneous Periodic Reports *Information for noted reports provided by Municipalities/Contractor/Consultant to CTDOT

REPORTING FORM 1585 One-Time Information Required From Each: Prime Contractor – Prime Consultant – Municipality: • Project Number/Town • DUNS Number • E-Mail Address • Submit Completed Form to District Designee Within 5 Days After the Preconstruction Meeting • District Designee Forwards Copy to: • Ted Szymanski (theodore.szymanski@ct.gov)

REPORTING FORM 1585 Monthly Information Required From Each: Prime Contractor – Prime Consultant - Municipality • State Project Number • Town • DBE Total Payments to Date for DBE’s Utilized by You • Form Submitted to District Designee by the 3rd of each month • District Designee Submits completed 1585 to • Ted Szymanski (theodore.szymanski@ct.gov)

REPORTING FORM 1589 Monthly Information Required From Each: Prime Contractor – Prime Consultant - Municipality • Labor Information for their firm and Labor Information for their respective subcontractors • Labor Information to be Reported: • Total number of employees utilized during the noted timeframe • Total Work Hours charged to the ARRA project • Total Labor Cost • Form Submitted to District Designee No later Than the 3rd of Each Month
