Results • Improvement of documentation in all areas • DNAR Documentation 53% to 100% • Discussions with family 97% to 100% • Instruction for care • Discontinued meds 39% to 81% • Palliative medication 27% to 90% • Withdrawal of organ support 57% to 87% • Referral for organ donation 11% to 87% • Improvements in spiritual and bereavement care
