Evaluation 58% increase in referrals for older people 68% of those admitted to intermediate care following hospital discharge return home at the end of their placement. 22% reduction in care needs through the effects of reablement. 89 fewer LTC beds purchased now than in March 2010. 66% improvement in the Balance of Care. Reduction of bed nights and ‘delayed discharges’ from hospital.
