Why Considerations are so Important in People’s Lives - Guy • People feel that they have been listened to, respected and valued • Real important change can occur for a person • People are often vulnerable and change is not common • People in the person’s life see improvement (Supports Coordinators, family, providers/others) • Individual change can foster system change

Considerations • Names have been changed to protect the identity of the person • Some considerations I shortened as the writeup and background material were lengthy, and I took dates out • I decided on what considerations to share from the many sent to me (lots of good ones) • Considerations come from 2017-2018 & 2019

  8. 3. Erie County Consideration Sharing • Background: A gentleman of which whom we will refer to as Jerry is in his early 60’s. He lives alone in a large apartment complex. He is quite content not working in the community because he keeps himself busy by visiting the Mental Health Association and various soup kitchens. He does not drive and relies on walking for his main mode of transportation.

Erie Continued • Consideration #1 Jerry would like a cell phone. • Consideration #2 Jerry would like to learn how to use a cell phone. • Consideration #3 Jerry would like a bus pass. • Consideration #4 Jerry would like a CD/radio player. • Consideration #5 Jerry would like to move to an area that he used to in the past.

  10. 3. Erie Continued • Response Consideration #1 SC met with Jerry at his home to discuss this consideration and others. They called the MECA Representative Payee, who authorized the purchase of a basic cell phone that will only provide calling features. Jerry has never had a cell phone before and is satisfied having a basic cell phone at this time. • Response Consideration #2 SC met with Jerry and discussed this request. Once Jerry receives the basic cell phone, SC will assist him with the initial phone set up. He will only make and receive calls. The basic phone will not have text capability. • Response Consideration #3 SC met with Jerry at his home to discuss this consideration and others. SC called MECA Representative Payee who will send Jerry a bus pass in the mail each month that he can use for transportation.

  11. 3. Erie Continued • Response Consideration #4 SC met with Jerry to discuss this request. Jerry will shop around and seek a price for a portable CD/radio player and notify the MECA representative payee who will authorize the payment for the player. • Response: Consideration #5 SC met with Jerry in his home to discuss this consideration. Jerry would consider living at another place, not only where he is now, but other locations. Jerry has agreed to stay at his current apartment during the winter and begin searching for a different apartment in the spring 2019. He would like to live in the city of Erie, because he eats meals at local city churches and City Mission. • Outcomes: Jerry is now able to have better access to the community through using his bus pass. He is able to have more communication through the use of his cell phone. Having a cell phone also improves his safety. He is able to have a higher quality of life having entertainment via his cd/radio player. His individuality is being respected for what he wants and what he values in his life to improve the quality of his life.

  12. 3. Fayette Consideration: Staff would like to see Patty go on a Disney Vacation. It may be possible to link the family/home/staff up with the variety of ID Vacation specialty companies that would help provide a goal for Patty to travel in the future if possible. Resolved- It was reported by the SC that she enjoys vacations and attended Disney in 2018. She has been there twice. Consideration: Karen stated she would like to buy a ring. It is my understanding that Karen loves rings. Resolved - She has gotten new rings as a result from her IM4Q interview.

Philadelphia • Consideration: Tim said he would like to visit his brother in Arizona and that he would like to take a trip to China one day. • Response: He and the brother’s ages are about twenty years apart and are not really that close, according to ‘Tim’s sister’ and have never lived together.  He joined the air force before Tim was born; he is now retired and lives in Idaho. Tim does have his address and now sends him cards several times a year. Tim is satisfied with this.

  14. 3. Philadelphia • Response: SC also discussed taking a vacation to China with Ms Smith. Ms Smith said she had never heard Tim express any interest in going to China. SC shared that Tim said he was asked what country he would like to visit. When asked why he picked China. Tim indicated that he likes the architecture, the culture, and the food. The team discussed exploring this request on a smaller scale. Timmy will work with staff and plan a trip to China Town in Philadelphia. The architecture, atmosphere, museums, and restaurants will offer a look into the Chinese culture. Tim was extremely excited with this plan. Tim and his staff went for a day trip to visit China Town in Philadelphia. Tim reported that he had an excellent day. Tim explored the shops, had lunch in a nice restaurant (tried to use chopsticks), and walked around and looked at the architecture and murals. Tim will visit the area again in the fall.

  15. 3. Armstrong/Indiana • We had three considerations for an individual, generated from the F/F/G interview with his sister. She is very pleased with how they were responded to by the Residential Provider Agency. She can’t say enough about how they responded and accommodated both her and her brother in his move. She knows her brother well and knew that if things aren’t presented to him in the correct way he can be very temperamental and resistant. She feels very strongly that the way the Residential Provider agency accommodated her and her brother, the transition went well. • Some of her concerns included: • Due to not having enough staff, her brother was not receiving the highest level of care that he had been approved for in 2017. • His declining health meant that he would end up in a wheel chair, how were his needs and his safety going to be met in his current home. • He had been in this home for a long period of time, so they were reluctant for him to move. They loved his staff and knew they did the best they could to meet his needs.

  16. 3. Armstrong/Indiana Included in the transition: • At the change of each season his sister comes to help him change out his clothing for the new season. So she arranged for a visit with the intent of helping with this and also speaking with him about moving into a new home. • She also told her brother they would be going on an adventure, so every day he would ask if today was the day they were going on an adventure. • Instead of weekend visits, his sister and provider agency arranged for lunch visits. It helped that he knew one of the individuals living in the home. • This individual told the person that they had a room and a bed available if he wanted to spend the night, he replied no he had his own bed. • During one of the lunch visits her brother said that he would like to have pancakes for lunch, so the staff said that if he spent the night he could have pancakes for breakfast. So he agreed to spending a night. • He was also told if he spent the night he would be riding home from program in the provider wheelchair van like his brother has, which he loves. • He then agreed to have his sister bring his new season clothing so they could put them away. • The next morning his sister noted to him that the walls in the room had nothing on them so they would have to go on an adventure to buy new pictures. • He was excited, it was time for them to go on their adventure.

  17. 3. Berks • We did a survey for a woman who lived on her own. She also has a boyfriend. She asked him to sit in on her survey. We entered a consideration for her stating she wanted to be able to move into a community home with her boyfriend. They both wanted to move into a community home. They both needed a little more assistance with managing their health, and they wanted to be able to live together. One of the community homes, that also happened to be their provider of choice, had a community home with a bedroom licensed for 2 people. The couple was approved for consolidated funding, and they were both able to move into the community home together and share a bedroom.

Northumberland • Consideration: Individual stated that he was having trouble sleeping because there was something wrong with his bed.  When he showed the monitors his bedroom, they could see that there was a problem with his bed so they wrote a consideration stating that the individual might benefit from having a new bed.  • Resolution -The following month he was shopping for a new bed.  I understand that it has been delivered and that he is sleeping better now.    • Consideration: Individual stated that he wanted to spend more time at home rather than attend his day program every day.  • Resolution - As a result of this consideration, he is attending day program three days a week and spending two days at home w/staff.

  19. 3.Bedford-Somerset, Cambria, Blair, Centre • Major Concern: Person had lots of problems with broken things at apartment, with sparks from outlets when plugging things in that landlord would not fix and something needed to happen • Resolution - Moved, and she and her fiancé are renting to own and the house has no safety hazards

  20. 3.Bedford-Somerset, Cambria, Blair, Centre • Consideration: Woman needs financial assistance for her daughter’s childcare as she can not afford it. • Resolution - She started a new job at a school cafeteria and no longer needs childcare as she starts work after her daughter leaves for school and is home before her daughter gets home.

  21. 3.Bradford/Sullivan Consideration: Individual stated she would like to work to become and work as a Zumba instructor. Response - Individual is currently training to be a Zumba instructor using services both from OVR and the ID waiver program. She has mastered 5 of the 10 routines needed for certification. AE/SCO Update: Individual is now trained and certified. She is now employed at two locations to instruct Zumba classes.

Bradford/Sullivan • Consideration: Individual would like assistance in finding avenues to pursue his interests in art paleontology. • Response: SC discussed with Individual, his mother, and Program Specialist about expanding Ed’s days to include a day that would focus on paleontology, starting with taking one of his Fridays to explore this and if he is interested then adding another day to his program that would focus on it. • AE/SCO Update: Individual attends the Susquehanna River Archeological Center (SRAC) every Friday to explore Paleontology. • Update since then—he is now volunteering at SRAC and has had drawings hung in the building.

  23. 3.Bucks • Consideration: Monitors feel that -------- needs some help with maintaining his apartment, has there been any discussion regarding an outside cleaning service. His health, eyesight and age could be a barrier to him being able to do this? • AE/SCO Response: ------- now has a cleaning company come into the home once a month to do a thorough cleaning of his apartment. -------- has also purchased an Roomba/iRobot to vacuum and keep his floor clean.

Bucks • Consideration: Family expressed that they need a Hoya lift and an accessible van can they be provided with information regarding how they might obtain these. ----needs to be lifted everywhere. Father broke his leg this summer. How have they managed!! • AE/SCO Response: Adaptive equipment was discussed at a home monitoring with the family. SC assisted the family in receiving adaptation for a van, County AE approved. • AE/SCO Confirmed Addressed?: Agree AE/SCO Confirmed Addressed Comment: SC did a great job assisting the family in obtaining the funding to purchase these items! Funding has been obtained and items are in the process of being ordered.

  25. 3. Lancaster • Consideration: A COMMUNITY JOB WAS MENTIONED BY BOTH KATIE AND DAD. • AE/SCO Response: Katie has begun the OVR assessment process. However, since it has been over 120 days since her intake, she is able to get supported employment services through Excentia (Katie's choice of provider). Katie will be receiving Job Finding and Development and Job Coaching and Support services through Excentia. Katie began employment with S. Clyde Weaver and now works 3 days per week, 6 hours per day. She continues to have check-ins from her job coach as needed. She and her team have reported that she has done very well adjusting to this job and enjoys it.

  26. 3. Delaware • Consideration: “S” and “S’s Father” wants to see a change in residential situation. Has S began to visit some residential facilities so that a choice can be made for a new residence? • Response: “S” has successfully moved into a group home with Devereux. • “S” has moved into a residential facility and is thriving at such facility. He has made new friends and is very happy in the new environment. His father is also very happy with this change.

  27. 3. Delaware • Consideration: “L” used to go to the YMCA for exercise, but had not been interested in going for a few months. Staff expressed that they would like to see her get more exercise. Is she resuming any regular physical activity? • Response: SC reached out to Residential team to discuss if “L” still has an active YMCA membership and if not to have this applied again. SC explained that during F/F monitoring that “L” was expressing how happy she was to be getting strong when at physical therapy and that this would be a good idea moving forward when her physical therapy ends. SC pending response from team on confirmation of her YMCA membership and how often she will go. • “L” has been attending the gym 2 to 3 times a week and is very happy doing so. In addition, to this assisting in keeping her moving, her visits to the gym is also a social outlet for her.

  28. 3. White Haven • Here are two considerations from Fiscal Year 2018-2019 that we received for two ladies who share a living area here at White Haven Center. Both considerations involve the primary theme of Relationships/Friendships. • Consideration #1 (for CF): Staff and the IM4Q team suggest this individual might benefit/enjoy contact with family via Skype, FaceTime, or via Web Cam. Staff noted that due to distance, her sister visits about once a year but calls weekly. • Consideration#2 (for CM): Staff suggests this individual might benefit from more family visits. If this is not possible, perhaps members of a community group (i.e. Senior Citizens, Girl Scouts, etc.) could stop in to see her.

  29. 3. White Haven • We started sending cards regularly to family members and friends this past October for Halloween. We sent them to the guardian and/or primary contact for each person. • We sent Thanksgiving cards to even more people…27 cards sent. • Many people received cards from family.

White Haven • 27 Christmas cards sent to family and friends. • We received 22 different family/friend contacts…cards, visits and phone calls. • March cards sent – 17 contacts received (calls, cards and visits). MP first the time ever received a Musical Happy Birthday Card from her mother. BP received a card and visit from Uncle & cousin. CF received a card from nephew J &his friend T.  PO (JF’s sister) called to talk to BC, CM, CF & JS on the phone.

  31. 3. White Haven • Valentine’s cards sent to friends, family and gents in living areas. • Easter cards sent - 21 contacts received. • Thinking of you cards were sent – 29 cards sent.   • See how two considerations helped a whole community connect, enhancing relationships on both sides of the card. • Fulfilling these considerations has left a positive impact, not only on these two women, but also on the lives of the other women who share the living area.

  32. State Center Consideration • Consideration - Woman resident wanted a magazine subscription • Resolution, as she is interested in food a food magazine subscription was ordered

  33. 3. Mercer • First consideration is a young man who is deaf. He had a light in his bedroom to let him know if someone wanted to come in, but there wasn’t a warning light to let him know if someone was at the front door. A front door light was installed and he felt more aware of the coming and going of visitors.  • Second, a individual wanted to be a veterinarian but didn’t want to go to school. She was set up as a volunteer at a local rescue and would go there to play with the animals. 

  34. 3. Northampton • Consideration:  Staff informed the team of this individuals love for swimming; he may benefit from an activity or vacation centered around this for his enjoyment. • Response:  P attends Beltsvile Lake, Lake Nockamixon and local pools during day program hours in the summer months. His CLA provider has recently purchased a membership to the YMCA so P can access swimming during the winter months. The YMCA has an indoor pool that P can enjoy a few times per week.

  35. 3. Lycoming/Clinton • Consideration:  H expressed a desire to learn more about self advocacy groups in his area. • Response:  H’s team met and discussed the IM4Q considerations. The team explained to him what self-advocacy groups are to ensure he really understood the question and wants to participate. H stated that he would like to attend a self-advocacy group. H and the team will identify available groups. The team offer them to H. The staff will take him to meetings so he can choose whether he is interested, and will provide necessary supervision.

  36. 3. Wayne- White Haven Center • Consideration:  Staff suggests Pat might benefit from finding ways to assist her in communicating more.  Staff reports she has the ability to communicate when she chooses to. Perhaps an evaluation by a psychiatrist, or speech/language/pathologist could help her communicate. • Response:  SC met with White Haven QIDP and Pat to review consideration. Staff suggested that Pat may benefit from finding ways for her to better communicate her needs. Based on an assessment completed by a Speech Pathologist, White Haven had obtained a communication device for Pat that will announce her name in a conversation. Recently, White Haven has obtained a second device and has been working on this device with Pat. The new device is more advanced than the previous device and is able to assist Pat in choosing preferred activities by giving her a choices to choose from. Team plans to continue to work on this device with Pat and test other devices as necessary. Pat indicated that she agreed with this plan by nodding her head.

  37. 3. Carbon-Monroe-Pike • Consideration:  Find a new IPAD to help individual improve his communication. He has been borrowing one from the Allentown Advocacy Alliance but it needs to be returned by mid December 2018 and he uses it all the time, everywhere he goes. • Response:  Michael has purchased an IPAD for himself. SC and SCS will locate appropriate applications that his waiver can provide for him for the IPAD.

  38. 3. Carbon-Monroe-Pike • Consideration:  Helene stated that she would like to be able to help cook in the kitchen more often at home and at day program. Helene may benefit from a cooking class. • Response:  Helene has been cooking at home as one of her goals this past year as per her ISP and she will decide if she would like to continue her cooking goal.

Carbon-Monroe-Pike • Consideration • Individual would like to learn how to vote • Response to Consideration • SC worked with individual and family to complete the process for voter registration. • Outcome of Closed Consideration • This individual is now a registered voter which has had a positive impact on her personal enhancement and growth.

  40. 3. Montgomery Consideration: • Parents requested Help/ assistance with changing the approved private school placement. • Parents requested assistance with gaining an understanding and working knowledge of the criteria for health coverage for their child. Response to Consideration: • The IM4Q monitor team visited the private home of a school age consumer. The parents shared that two of their children receive ID services and are registered with the county Office of Developmental Disabilities. Over the years, the parents tried to monitor the various ID system issues and updates and act as advocates for their children. As the children got older their medical and school related issues multiplied to the point that the parents were physically and emotionally exhausted and unable to effectively manage the day to day tasks much less mange the more complex issues of Transition Planning. Unfortunately, both parents acquired chronic health conditions as well as suffered loss and reduction of their employment status and wages. As they slipped “deeper into a black hole” they were less able to manage all of the details of their two children. Both the stories that the parents shared with the IM4Q team and what the team observed indicated that help was necessary to protect the family. The IM4Q team documented the parent’s details of their struggle to manage the medical and school issues of their child. While the situation technically did not fall under a “Major Concern”, the team’s report was submitted promptly into HCSIS considerations.

  41. 3. Montgomery • The SC supervisor contacted the family the next day and visited with the family to assess the situation. In addition, the Director of Special ED from their home district was contacted by the parents and a meeting of all parties took place within 5 days to address the Approved Private School placement. With the help of the team, the family visited various placement options. The family selected a full time residential option for one of their children. • The family also contacted an advocate to help them understand the protocol and guide them through the process for resolving health care coverage and eligibility. Outcome of Closed Consideration • Since the initial IM4Q team visit, the family has been able to avoid foreclosure on their house since they were able to improve their health and ability to gain employment. Most importantly, they believe that the residential placement for their child was an important step to give their child the best chance at a self determined life and restore harmony in their family unit. Additionally, their other child has benefited from their increased attention. They are thankful that the IM4Q Team visit and Considerations which resulted in restoring their family unit and their confidence to parent their children. • They maintain contact with the Advocate and were satisfied that the specific health related issues were fully addressed and resolved especially regarding the criteria for health care coverage.

  42. 3. Montgomery • Outcome the Closed Consideration: • First the SSI and MA issues were resolved to the consumer’s benefit both emotionally and financially. Consumer developed additional skills in self advocating with employer on the issue of benefits. Consumer felt confident that ‘the support team’ was acting solely on the wishes of the consumer. Consumer was reassured of the importance of developing a circle of support comprised of the SC, family, neighbors and employer. Consumer and family developed skills to ask for and seek help in navigating the system both locally and state wide. • Secondly the cascading effect of having more disposable income increased the consumer’s choices and willingness to participate in community activities. Consumer was able to dine and shop in the community, which resulted in development of natural friendships. • Thirdly there is an improved relationship with family and consumer since the time that they spent together now is devoted to typical activities like; meals out, movies and community activities. Previously, both family and consumer became frustrated over the SSI and MA issues that each avoided spending time together. • Lastly consumer has become knowledgeable about SSI and MA issues and regularly shares the experience and process with others who are seeking advice. Consumer also joined a local advocacy group and shares the acquired knowledge based on personal experience.

  43. 3. Lawrence Consideration: A sister requested assistance for her brother to learn to cook simple meals. Resolution - The individual was able to get H&C to come over every other Saturday to learn new, easy, meals. He was doing so well, he gained more weight than he should have which led to educating him on healthy choices and enrolling him in an exercise program. He has lost 10 pounds to date, and feels much better.

  44. 3. Beaver Consideration: TIFFANY REPORTS THAT SHE IS BEING BULLIED AT PARC. THE BULLYING IS TARGETTING OTHERS AS WELL. AE/SCO Response: Tiffany, her Program Specialist and SC had a meeting at PARC where the incident with "bully" was occurring. Tiffany was able to inform her Program Specialist who the target was and that she had once been friends with this woman. They only see one another during break which is monitored by staff and also at the end of the day by the lockers. Tiffany agreed to wait 5 minutes before getting her items so that there would not be contact between the two of them. SC followed up with Tiffany and her mother/guardian to see how things have been going at PARC since meeting. SC informed that there has been no further Issues and that waiting 5 minutes has solved the problem.

Franklin-Fulton • Consideration:   The individual mentioned that she would like to see her family (who live in Florida) more often.  Could SC provide resources and information as appropriate? • Response - she is now planning a trip to go to Florida for a week the first week in June. she is going to fly out of Hagerstown airport and her sister will be picking her up from the airport in Florida. she is very excited about the upcoming trip. • Outcome:  The individual made the trip and with the coordination between the sister, provider, SC and the airport personnel, all went smoothly.  She would like to do so again.

Crawford • Consideration- Would like to have life skills work on more life skills and safety rather than just fun stuff. • Outcome: AT THE INDIVIDUAL'S ANNUAL MEETING THE TEAM DISCUSSED ADDING A "HELPING OUT AT HOME" OUTCOME TO HIS PLAN. SINCE THEN, HE HAS STARTED GOING TO THE LAUNDROMAT WITH HIS STAFF TO LEARN TO DO HIS OWN LAUNDRY. THEY HAVE ALSO IMPLEMENTED A CHECKLIST THAT STAFF HELP HIM CHECK OFF EACH MORNING HE HAS DAY PROGRAM. THE CHECKLIST INCLUDES: SHOWERING, SHAVING, BRUSHING TEETH, WEARING CLEAN CLOTHES, AND PICKING UP HIS ROOM. ROBBY KNOWS THAT IF THESE AREN'T DONE IN THE MORNING WHEN HIS STAFF ARRIVES, THEY WILL WAIT FOR HIM TO COMPLETE THEM BEFORE THEY GO OUT IN THE COMMUNITY.

  47. 3. Crawford • Consideration-Funding for transportation to see mother at Rolling Fields • Outcome: THE INDIVIDUAL AND THEIR TEAM DISCUSSED THIS AT THE INDIVIDUAL'S QUARTERLY MEETING. ROLLING FIELDS IS APPROXIMATELY 17 MILES AWAY FROM THE INDIVIDUAL'S HOME IN MEADVILLE, 34 MILES ROUND TRIP. THE INDIVIDUAL'S PAYEE ASSISTED THEM IN CONTACTING CATA TO SEE ABOUT SETTING UP SHARE-A-RIDE DRIVES TO VISIT THE INDIVIDUAL'S MOTHER. CATA INFORMED THEM THAT THE DISTANCE WAS TOO FAR, THEY WOULD NOT GO THAT FAR OUT OF MEADVILLE. AFTER LEARNING THAT THE SHARE-A-RIDE WAS NOT AN OPTION, THE INDIVIDUAL'S TEAM PUT PLANS IN PLACE SO THE INDIVIDUAL CAN SEE THEIR MOTHER ONCE PER WEEK. THE INDIVIDUAL'S LIFE SHARING PROVIDER TAKES HER OUT EVERY OTHER SATURDAY AFTERNOON. THE ARC ADJUSTED THE INDIVIDUAL'S MILEAGE CAP FOR DAY PROGRAM SERVICES SO THEIR CPS PROVIDER CAN TAKE THEM EVERY OTHER THURSDAY DURING DAY PROGRAM ON THE WEEKS THEY DOES NOT GO WITH THEIR LIFE SHARING PROVIDERS. THE INDIVIDUAL AGREED WITH THIS PLAN AND THEY WERE THE ONE THAT DECIDED THAT ONCE PER WEEK WAS A GOOD AMOUNT OF VISITS.

STAFF ALSO SAID THAT A FLOWER COULD BE PLANTED AT THAT TIME. THE TEAM AGREED THAT ANDREW COULD ALSO WORK ON COPING SKILLS DURING THE SITE VISIT. ANDREW WAS EXCITED AND AGREED TO GO WITH STAFF ON MEMORIAL DAY

THE PROVIDER STATED THAT HER ROOM WILL BE PERSONALIZED AND TRANSFORMED INTO A ROOM WHERE NOT ONLY DOES SHE SEE PERSONAL ITEMS BUT ALSO SEES SENSORY STIMULATING ITEMS. SINCE SHE RETREATS TO HER ROOM QUITE OFTEN AND IT IS BELIEVED THAT THIS IS WHERE SHE'S MOST COMFORTABLE. THE TEAM BELIEVES THAT GIVING HER A SPACE OF HER OWN AND WHERE SENSORY INTEGRATION CAN BE INCORPORATED WILL MOST BENEFITUAL. THE TIMELINE TO IMPLEMENT THIS CONSIDERATION IS SET FOR 05/01. THE TEAM IS SATISFIED WITH THIS RESOLUTION AND WISHES FOR THIS CONSIDERATION TO BE CLOSED.
