How to Enroll or Increase Coverage Open Enrollment begins at your Employee Self Service Site Self Service Link • Enroll on-line for the following benefits: • Life Insurance - up to the Guaranteed Issue Limits • for Employee, Spouse or Child • Long Term Disability • Enroll with a paper application for the following benefits: • Life Insurance above the Guaranteed Issue limit • for Employee or Spouse • Short Term Disability • Accidental Death and Dismemberment

The Optional Application Applying for Coverage When Health Questions are Required Optional Application Link Step 1) Submit your completed Optional Application to the address on the bottom of the form. Step 2) MMB will forward your application to Minnesota Life or Hartford Life, as appropriate. The insurance company will call your home and/or email you a health questionnaire with instructions. Step 3) Complete the questionnaire as instructed. Upon the insurance company’s receipt of your response, they will review your application and advise you of their decision. If approved, the State will also receive notification to begin payroll deductions.

Optional Term Life Insurance Minnesota Life • Policy Features: • Apply any time of year… Health questionnaire required for amounts greater than the guaranteed amounts offered this year • Employee & Spouse Maximum - $500,000 each • Cost based on age • Benefit doubles for accidental death • Premium waived to age 70 if totally disabled • Accelerated benefit if terminally ill • No cost post-retirement benefit Child Life: To age 26 regardless of student status • $10,000 per child • $0.42 / semi-monthly

  6. No Cost Post Retirement Benefit Minnesota Life No cost after 65… coverage is permanent for life! Example: Assume $100,000 is the smallest amount of coverage 15% of $100,000 = $15,000 no cost death benefit

  8. Long Term Disability Hartford Life • Benefit Features: • Benefits not taxed • Benefits integrate with other income (ie. Social Security & MSRS) • $300 minimum benefit • 2 year exclusion for pre-existing conditions RATES REDUCED by 10% this year - Enroll or Increase online, only during open enrollment • Benefits Begin • After 6 months of total disability • Payable • Up to Social Security normal • retirement age (age 65-67, • depending upon your year of birth) • Choose Your Monthly Benefit • Minimum $300, up to 60% of • gross salary • Maximum $7,000/month

Short Term Disability Hartford Life • Benefit Features: • Benefits not taxed • Paid in addition to sick/vacation • RATES REDUCED by 2% this year - Enroll or increase at any time, no health questions – paper application required • Benefits Begin • 1st day of an accident • 8th day of an illness/maternity • Payable • Up to 26 weeks • Choose Your Monthly Benefit • Minimum $300 up to 66 2/3% of gross salary • Maximum $5,000/month
