Ladder Policy Update • Ladder policy revised 2/28/2018 • Anytime you are using a Ladder you must fill out the Ladder SWP. • If you can not maintain 3 points of contact on the ladder, you MUST tie-off. • If you are going to be working from a Ladder for more than 30 mins. You MUST contact your supervisor and reevaluate the job.

  16. Background info Every year, some of our employees, contractors and others around us lose their balance while climbing up/down stepladders /portable ladders or working from one. Potential consequences of related injuries are usually severe: fractures, cerebral concussions, up to possibly a fatality in our sites. A true choice of zero injuries in our sites rests first and foremost on the effective and rigorous management of risks that are identified and need to be controlled. This is not a wish, but a business choice that entails that employer’S REPRESENTATIVES have the responsibility and ultimate duty to ensure our employees and people ON our sites are protected against work-related hazards. Two (2) of our employees recently lost their balance (on February 7 & 23) as they were climbing down a stepladder Or ladder at a height of ≈4-5’, AS they were NOT maintaining effective 3-point contact. Result: severe contusion and cut/fractured wrist.

  17. Background info The choice of “0” injuries in our sites rests first and foremost on effective and rigorous management of identified risks that need to be controlled. To control fall hazards when ladders/stepladders are used, It has become imperative to raise the risk reduction level from administrative control To the levels of consequence Reduction and FOCUS FrequencyReduction As often as possible, And to not tolerate any Deviations from our safety administrative requirements (when these equipment are being used because it is the only OPTION available for a specific job)

SAFETY ACCESS RULES FOR AJUSTABLE PLATFORMS Maintain 3 points of contact ALL TIME to climb up or coming down, ALWAYS facing the portable ladder/stepladder Apply firm grip (non-slip gloves) by holding the platform rungs when climbing up or coming down NO TOOL OR OBJECT IN HANDS when climbing up or coming down. If tools are required, they MUST be carried on in a tool case or lifted within an appropriate container once they reach the required height for the task. Hazard Control Levels for Working at Heights • Note: Workers who use a safety harness must: • Be in good shape • Visual inspect harness before every use • Select equipment that: ▪ is fully compatible, • designed for the type of job at hand, and • reduces fall distance to a minimum • Never use equipment intended to be used for material hoisting. Grab safety harness by the back D-ring and shake it to untangle the straps. Slip on shoulder straps and ensure D-ring is centered between your shoulder blades. Bring leg straps up between your legs and connect the buckle.

  27. Hazard Control Levels for Working at Heights Slip your hand down to the knuckles under every strap to ensure they are not too tight. A well-fitted harness must allow you to move freely. Adjust shoulder straps, making sure they are of equal length. Attach chest strap firmly but without hindering your breathing (you must be able to slip your hand down to the knuckles between your body and the strap). Attach lanyard to the back D-ring, making sure it does not hit the back of your head. To maintain harness effectiveness, ensure none of the straps are twisted. Ask for a co-worker’s help to check straps in the back.

  32. step 2: selection • Before selecting a ladder or stepladder, the following work characterization elements must be taken into account: • Type of job • Work environment • Work height • Material and • Rated capacity The choice of equipment’s material is a determining factor when selecting equipment. A job risk analysis allows you to select one material or another.

  33. step 3: inspection Stepladder Ladder End caps are free from cracks, chips and wear. ID labels are in place and readable. Top platform and rails are free from cracks and dents. Clearance is at least 6 inches behind the rungs. Rails are straight (not twisted or warped) and in good condition. Fasteners (bolts, nuts, etc.) are in good condition and tight. All rungs are present and solidly secured (not loose), no dents or cracks. Rungs are clean and non-slippery, free of mud, oil, snow or ice. ID labels are in place and readable. Fasteners are present and tight. Steps, step braces and horizontal braces are in good condition. Spreader arms move freely and lock properly. Examine fiberglass ladders closely for any breaks, cracks or nicks. Note: Inspect stepladder before every use and ensure its various elements are clean, in good condition and free of any defects. All 4 safety feet are solidly secured, with non-skid shoes in place and in good condition. Feet on both rails are equipped with non-skid shoes, and braces are in place and well tightened. 41

  34. step 4: EXECUTION OF WORK Ladder Ensure areas around the top and base of ladder are clear of any obstacles. Secure ladder firmly before extending it. Never install it while extended. Set up ladder at the correct angle. First, assess distance between the upper and lower support points (L = Length)… then move base away from the top support by a distance of 1/3 to 1/4 of this length (cf. image). Stepladder Open the stepladder spreaders and shelf fully. Lock spreaders in place. Ensure all 4 feet are equipped with anti-skid shoes. Check stepladder for stability before climbing up. Ensure stepladder is set up near the work surface.

Check for stepladder stability before climbing up Make sure the stepladder is being positioned close as much as possible to the job

  37. SPECIFIC MANDATORY EXPECTATIONS Chose an appropriate ground surface, firm and non-slippery. Ladder base must be perfectly level and non-skid shoes in full contact with solid ground. Never position a ladder or stepladder in front of a doorway, emergency access or firefighting equipment. Use a ladder to circulate or access a compliant work platform. Make sure to FULLY applythe 3-point contact rule while climbing up/down or working without fall protection; two hands and one foot or one hand and two feet at all times when climbing up / down or working. Always face the ladder and/or stepladder, holding on to the rungs/steps rather than the side rails as they allow for a better grip. Only one person may climb up or down at a time. Never climb up to one of the top three rungs of a ladder. Never stand on the last step of a stepladder to work. Keep body centered between the ladder/stepladder rails, never extending outside the rails. IF NOT, you MUST climb back down and reposition the stepladder or portable ladder. When working , ALWAYS face the ladder/stepladder
