E-Care Health is a digitalised health care service instituted by CBN Digital Health Care Foundation. CBN Health Care Foundation, a NGO was founded by Shri. Venkat Raghuveer Rallabandi, his Wife Smt. Shyamala Sridevi and shri. D Venugopal with an intention to serve the people with the best health care services. E-Care Health is collaborated with highly qualified and experienced Doctors, Nurses, and Healthcare professionals across various Hospitals and Pharmacies to establish quality healthcare ecosystem to all. With E-Care Health being associated with more than 200 Doctors and 10 Hospitals in Hyderabad, we aim to reach a target of over 1 million E-Care Health members by the end of 2019.<br>

E Care Health Center • E CARE HEALTH Centre would be located within 3 Km radius in any area of hospital or clinic which ensures to bring more footfalls to your clinic/hospital. • E CARE HEALTH Center will have • Qualified medical attendants • Qualified trained nursing staff 24X7 • Ambulanceswith ALS(Advanced Life Support) features. • 24 X 7 service backed up by centralized call center with 100 professional trained and qualified call center executives. • Digital health profile of the patients will be created and maintained with 100% confidentiality. • Ensure to provide/ supply the best and branded medicines from core manufacturers to all the members.

Hospitals • We offer a tie up with hospitals from • 50:50 to 80:20 ratios of occupancy. • Doctors, Surgeons, and Specialists will be on our rolls based on market standards. • Providing medical equipment and devices as per the requirement. • Trained and untrained nurses will be on our rolls and • Each State will be backed up by 100 ambulances. • All other Government schemes can be applied and operated as usual by individual hospitals. Responsibility of Hospitals • Hospital need to • Make on time payment of rentals. • Hospital Maintenance including • Manage administrative, housekeeping and all supportive functional Staff including security on its own. • ensuring uninterrupted supply of power and water supply • Proper hygiene should be maintained. • Payment of periodical Government levy’s and any local taxes. • Submission of reports and audits as per norms of CBN. • Disclose of all the tie-ups with the third party, if any. • All the transactions are subject to GST and Income Tax deductions Back

Doctors • Doctors will be associated with E-Care Health centers situated with in a radius of 3 kmshence reducing the travel time and optimizing utility of potentials. • Commitment of 100 to 150 patients every month per Doctorwhich will ensures secured income of Rs.1, 00,000 to Rs. 1,50,000 p. m based on experience and expertise. • Doctors will be part of all the Hospitals, where the organization has a tie up. • Doctors will enjoy the privilege of association with CBN foundation. Members / Patients What a Member Gets in 3 Years • Complete Digital Medical Record • Rs.4,000/- worth of Medicines as per the prescription of the Doctor. • Rs.4,00,000/- worth of Coverage for Operations of any kind per year, except Cosmetic Surgeries {no accumulation allowed, no carry forward is allowed}. • Membership cannot be transferred or No proxy. • Free Pick up and Drop facility in case of hospitalization. • Home Care for Aged people and Post Operation care, Free nursing care for 5 days. • For Pregnant women from 7th Month delivery expenses, medical expenses of Mother and Baby will be covered till the New Born Baby attains ONE Year. • 6 times hospitalization expenses are covered in a Year. • Membership fee Rs 8000.
