    2. Palmetto Health Baptist CME Program Accredited by the South Carolina Medical Association to provide AMA PRA Category I credits™ through the national Accreditation Council on Continuing Education (ACCMEŽ). PHB CME has received the highest accreditation recognition available for CME programs. In 2009, PHB CME received, Accreditation with Commendation status (6-year). The first CME program accredited through the SCMA to receive this status. The mission of Palmetto Health Baptist Continuing Medical Education Program is to provide physicians and stakeholders with the relevant strategies and skills needed for professional development and to improve patient care.

    14. Performance Improvement CME (PI CME) 5 + 5 + 5 = ?

    15. Performance Improvement CME (PI CME) 5 + 5 + 5 = 20 AMA PRA Category 1 Credits™ Performance Improvement Activity For physicians designing and implementing a performance improvement project in their practice.

    16. PI CME PI CME consists of the four parts: 1. PICME Application 2. Stage A = 5 AMA PRA Credits™ 3. Stage B = 5 AMA PRA Credits ™ 4. Stage C = 5 AMA PRA Credits ™ * Once Stages A, B and C are complete, you will be awarded. = 5 AMA PRA Credits ™

    17. PI CME STAGE A: Learning from current practice performance assessment Assess current practice using identified performance measures, either through chart reviews or some other appropriate mechanism. Participant physicians should be actively involved in data collection and analysis.

    18. PI CME Stage B: Learning from application of PI to patient care Implement an intervention based on the performance measures selected in Stage A, using suitable tracking tools (e.g., flow sheets). Participating physicians should receive guidance on appropriate parameters for applying an intervention and assessing performance change, specific to the performance measure and the physician’s patient base (e.g., how many patients with a given condition, seen for how long, will produce a valid assessment?).

    19. PI CME Stage C: Learning from the evaluation of the PI effort Re-evaluate and reflect on performance in practice (Stage B), by comparing to the assessment done in stage A. Summarize any practice, process and/or outcome changes that resulted from conducting the PI activity.

    20. PI CME Performance Improvement (PI) CME projects are structured, long-term processes. Physician or group of physicians can learn about specific performance measure and retrospectively asses their practice, apply these measures and re-evaluate their performance. Measures must be evidence based and well designed (e.g., clearly specify required data elements, data collection is feasible). PI activities may address any facet (structure, process or outcome) of a physician's practice with direct implications for patient care.

    21. PI CME Each PI CME Application must be reviewed and approved through PHB CME Committee. Application available on the CME Website. www.palmettohealth.org/phbcme/forms

    22. Internet POINT of CARE (PoC) - Structured, self-directed, online learning by physicians on topics relevant to their clinical practice. Driven by a reflective process in which physicians must document their clinical question on a short Survey Monkey questionnaire. Sources referenced should be from approved reference sites as listed on the PHB CME website and available through PHDOC.

    23. Internet POINT of CARE (PoC) Participating physicians must: (Easy access and documentation through PHB CME website.) Document original clinical question(s). Identify the relevant sources from among those consulted, Describe the application of their findings to practice. Physicians conducting structured online searches on clinical topics may claim (0.5) AMA PRA Category 1 Credit™ for documented completion (either at the point of care or later) of the three step learning cycle defined above. Easy to fill out PoC forms on the CME website with links to approved research sites, such as ZynxEvidence, (available through PH Doc.)
