What Should the Member Expect? • Trained clinician performs screening • Member receives results: • an incentive gift • Member receives personalized health education: • brochures • videos available for viewing • opportunity to sign-up for provider’s classes (weight loss, adult fitness, smoking cessation, etc.)

  10. What Should the Member Expect? • Results are forwarded to member’s physician • Member can create an individualized Health Status Report via the Internet with unlimited updates • If Internet access is not accessible, a questionnaire will be sent to member
