Case of the week: Number 06-02. High Risk HCM Case from: Carolyn McCarthy, Perry Elliott, The Heart Hospital, London, England History: An 33 year old man with HCM with a family history of sudden death. Seven years previously, a single Holter had shown 3 beats of NSVT. Cine CMR shows HCM with ASH, no SAM, no LVOTO (left or right), and normal systolic function. After gadolinium, there was very extensive late enhancement, (44% of LV myocardium, arrowed). The scan triggered re-evaluation of cardiac risk, and the patient was offered an ICD. Conclusion: Extensive late enhancement indicating scar in HCM, particularly at a young age, is associated with clinical risk markers.
