7. Care Everywhere (Care Epic) • Dane County Care Everywhere Coalition Members • Epic Software Allowing Members to Securely Exchange Patient Information • Launched March 29 in Emergency Departments and Urgent Cares for 4 Month Pilot

8. Early Exchange Results (First two weeks) • Average 113 patients daily with a Care Epic exchange • Frequency of exchanges varied by Madison hospital

UW Health MyChart • A secure online service that offers users the privacy and convenience of accessing portions of their UW Health medical records. • Allows you to: • Receive test results online • View and print selected health issues, medications, allergies and immunizations • Schedule, cancel and view appointments • Send secure electronic messages to your health care team • Access billing information • Access the above information for your children or others you care for (authorization required/age restrictions apply) • UW Health Primary Care Clinic patients are encouraged to visit uwhealthmychart.org to "Request an Account." An activation code will be mailed to your home in 7 to 10 business days.
