  3. Who We Are • Specialized expertise • Post-hospital rehab • Integrated, individualized care • System navigation • Care giver support • Consultation and education

  4. Who We Are • 3 Teams serving Thames Valley, Huron Perth, and Grey Bruce regions • Comprehensive group of rehabilitation professionals: Nursing, Physiotherapy, Occupational Therapy, Social Work, Speech Language Pathology, Therapeutic Recreation, and Rehabilitation Therapy

How We Interface with Clients • Our program is designed to help stroke survivors who are no longer in hospital to reach their rehabilitation goals. • A stimulating environment promotes greater functional recovery. • Our team focuses on helping people return to activities that are important to them and/or helps them discover new meaningful activities.

  6. How We Interface with Clients • We deliver stroke rehabilitation to clients and caregivers in their own community. • This means providing education and therapy in a variety of community settings: • Private residences • Long term care Homes • Community centers

  7. How We Interface with Clients • Education, coaching and developing problem solving skills: improve quality of life, lessen care giver burden, and reduce stroke risk factors. • Ontario Government’s Aging at Home Strategy to help seniors live healthy, independent lives in their own homes.

How We Collaborate with Others • We provide education and consultation to other service providers on how to better care for stroke survivors. • Tips and Tools, clinic days, consultation • Support groups: Living with Stroke, Stroke Recovery Association partnership, LHSC/SJHC joint support group

  9. How We Collaborate with Others • Ongoing synergy between CSRT and other community agencies, e.g. CCAC • Coordinated effort, not duplication or elimination • Working together for efficient use of resources: • More people can be served – increase capacity for existing programs • Decreased wait times for existing services
