Cyclophosphamide Cyclophosphamide is part of the FCR (fludarabine, cyclophosphamide, rituximab) regimen, which is generally considered the first choice treatment for younger/fitter patients with CLL. It is also used in combination with the purine analogue pentostatin in the PCR (pentostatin, cyclophosphamide, rituximab) regimen. In older patients or those with comorbidities it can be combined with prednisone with or without rituximab. Additionally, cyclophosphamide is a component of the following regimens used to treat patients with CLL: • RCHOP (rituximab, cyclophosphamide, doxorubicin, vincristine, prednisone) R-HyperCVAD (rituximab, cyclophosphamide, vincristine, doxorubicin, and dexamethasone alternating with high-dose methotrexate and cytarabine) • dose-adjusted EPOCH-R (etoposide, prednisone, vincristine, cyclophosphamide, doxorubicin, rituximab) • CFAR (cyclophosphamide, fludarabine, cytarabine, rituximab)
Chlorambucil Chlorambucil is an oral agent that is primarily used to treat older or less fit patients who cannot tolerate more aggressive agents. It can be used alone, or in combination with rituximab.
