  3. Enter information in the required fields Remember that your Facility Number and FIN are printed on the mail out that you received from DSHS in September and/or December 2010 Next, click the Submit button

  4. Next, click the Continue button Write down your User ID and Password. You will use the same User ID and Password to login to the system next year.

  5. Enter the User ID and Password that you wrote down Next, click the submit button to login

  6. This is the end of the CHRS Login Tutorial. Data entry instructions for the Annual Report of Immunization Status and the Vision-Hearing-Spinal Screening Report are located in separate tutorials on the CHRS home page.
