Shawnee Health Service Operates 3 major Programs: Community Health Center Program at 10 sites in a two county area 3 FP/OB clinics 1 Adolescent clinic 2 School-based clinics 1 mental health clinic 2 Respiratory clinics 3 Dental clinics 10 Family Practice 2 OB/Gyn 2 Internal Med/Pulmonology 10 Mid-levels 4 Psychiatrists 4 Dentists Population served: 19,109 users 89,346 encounters Mostly underinsured or Public Aid Southern Illinois/SW Indiana Respiratory Disease Program Shawnee Alliance for Seniors

Shawnee’s School Health Centers Terrier Care: • located at Carbondale Community High School • Serves District 95 (1351 student + 104 faculty + families) District 165 (1161 students and 72 faculty + families) Marion Wellness Center: • located at Marion High School • Serves District 2 (pre-k – 12) 4068 students + 217 faculty + families

  5. How Marion Unit 2 School District Evolved • School Administration Initiative • Hired Consultant • Renovated space for medical services at the High School • Recruited a medical provider …..and Shawnee Health Service enters the picture……..

Marion’s Major Milestones • July 2004: Shawnee Health Service enters into discussions • January 17, 2005: Opened SHC with medical services • August 2006: Opened dental operatory at the SHC ($205,550 ILCHF grant for MWC and AHC) • September 2007: Started Dental Sealant Program (IDPH grant) • October 2007: Initiated Caring Connections Network ($80,000 IDN grant) • August 2008: Counseling Caravan will begin ($100,000 ILCHF grant to community mental health agency)

Collaborative Partners forMarion Wellness Center: • Southern Illinois Healthcare: Illinois Delta Network grant funding ($12,500) • Illinois Children’s Healthcare Foundation: Dental grant • Franklin Williamson Human Services • Southern Illinois University: Illinois Delta Network grant funding • Illinois Department of Public Health: Dental Sealant grant (billed per exam/sealant)

Medical Visits: 2004/2005: (3 months operation) 375 visits 2005/2006: 1525 visits 2006/2007: 2148 (41% ) 2007/2008: 2456 (14% ) Dental Visits: 2006/2007: 625 2007/2008: 890 (42% ) Marion’s Utilization – • Patient Satisfaction: • 100% of patients surveyed would recommend Marion Wellness Center to their • friends and relatives • Over 67.5% listed Marion Wellness Center as their main source of medical care

  9. Strengths and Weaknessesof our relationship with the School: • School very committed to project and its success • School system is very closed to “outsiders”; difficult to bring in new partners. • Requires extra time for school administrators to ok projects

  10. How Carbondale School District Evolved • Wellness Committee completed CDC’s School Wellness Index • Sought medical provider to provide care to uninsured/low income (Shawnee invited) • Location for Health Center incorporated into school nurse office/renovations completed

Carbondale’s Major Milestones • August 2004: Shawnee Health Service enters into discussions to become medical provider • September 2004: Secured planning grant funding from DHS ($25,000) • March 2005: Opened SHC with medical services • July 2005: Secured on-going DHS funding ($100,000 annually) • August 2006: Opened dental practice at the nearby adolescent clinic (ILCHF grant) • August 2007: Expanded clinic from 650 – 1200 square feet (ILCHF grant, Fralish Family foundation and SIH) Strengthened relationship with local mental health agency for counseling services Added District 95 as eligible users • January 2008: Started on-site mental health services (Southern Illinois University doctorate level psychology program)

Collaborative Partners for Carbondale’s SHC: • Wellness Committee • Jackson County Health Department • Southern Illinois Healthcare (Illinois Delta Network $12,500; expansion project $25,000; & on-going program support directly awarded to the school) • Southern Illinois Regional Social Services (mental health services) • Southern Illinois University (Psychology program) • Illinois Department of Human Services (planning grant & on-going School Health Grant) • Fralish Family Foundation ($10,000 expansion grant)

Medical Visits: 2004/2005: (2 months operation) 147 visits 2005/2006: 947 visits 2006/2007: 1111 (17% ) 2007/2008: 1312 (18% ) Mental Health Visits: SIRSS: 43 students in Fall semester/21 open cases into the spring Psychology students: 29 students/77 counseling sessions in Fall semester/12 open cases into the spring Carbondale’s Utilization – • Patient Satisfaction: • 100% of patients surveyed would recommend Terrier Care to their • friends and relatives • Over 60% listed Terrier Care as their main source of medical care

  14. Strengths and Weaknesses of our relationship with the School • Great “grass roots” support in the school – teachers, principal • Great support from other community health agencies in the area – monthly meetings

Lessons learned: • Consider your community’s uniqueness. • Identify your community strengths – health departments, mental health agencies, hospitals. • Understand your school’s interests in the management of the program. • Engage school staff in the promotion of the school health program. • Identify funding sources – locally, state, or federal that can help you start and/or sustain your programs.
