Questions to Consider 4. How will the QIT work with program personnel around implementation of CCFC? • Team members? • Consumers? • Families and significant others? • Other agency program managers and staff? • Others (e.g., Board members)?
Questions to Consider 5. How will the QIT ensure that CCFCs are occurring and who will be responsible for collecting and maintain data?
Questions to Consider 6. How will the CCFC model be made available to consumers (e.g., intake procedures, staff review of caseloads)?
Questions to Consider 7. How will others outside of the agency (NAMI, NAMI Court, etc.) be made aware of the fact that CCFC is available? Reviewed so far: Leadership, QIT team, engagement of stakeholders and performance indicators. Any questions or other thoughts?
