  2. FORM 1006B – OPERATORS AGREEMENT TO PLUG OIL, GAS AND SERVICE WELLS WITHIN THE STATE OF OKLAHOMA This presentation will assist you in the completion of the Form 1006B. The operators agreement to plug oil, gas and service wells within the State of Oklahoma. Date of Last Revision: 2010 Rule Requirement: OAC 165:10-1-10 Explanation of Form: Operator shall agree to plug well(s) in compliance with the Commission rules. This agreement must accompany the operator's elective choice of surety (Form 1006, 1006A, or 1006C). The operator is required to file a Form 1006B with the Conservation Division once every twelve (12) months. [Reference 165:10-1-10, 165:10-1-11, 165:10-1-12, 165:10-1-13, and 165:10-1-14] Question concerning this Form: Contact the Surety Department (405) 521-2246

New Operators that are filing for the first time, leave this space blank and the surety section will assigned you an operator number
