Steps toward achieving goals: Identify ideal membership of CLT, define mission and goals of CLT Determine process for adding new members Develop invitation to send to members Determine meeting logistics Implement the plan Established roles and expectations for the CLT Defined scope of work of CLT Developed measure of progress of SHIP Developed one-page recruitment letter for potential CLT members Brainstormed potential recruits to the CLT Conducted Healthy communities stakeholder analysis Conducted SWOT analysis for coalition building • Milestones achieved • Members understand the purpose of the group and roles and expectations. • Generated significant interest with community partners regarding SHIP • Collected 41 letters of support from the community for the RWJ project on Healthy Kids, Healthy Communities • Recruited coalition members to participate in the SHIP policy training workshop • Standardize the improvement • We achieved our Aim and each of our measures. • 16 partners signed letters of commitment to participate in the HCC. • Letters of commitment include 4 hospitals, 3 public health departments and the University of Minnesota Extension Service. • Community engagement is about relationship building - to keep members engaged, everyone has to be benefitting from participating on the collaborative • Quality improvement tools are a great way to give new direction to a collaborative that is having a hard time identifying how to proceed PLAN • Problem • Lack of a formal leadership structure to implement the Statewide Health Improvement Program • Aim • By June 30th, 2009, the MMS CHB will develop a working community leadership team (CLT) within the existing Healthy Communities Collaborative [HCC] in order to broaden the sector participation: schools, communities, healthcare and worksites. • Goals • Draft a recruitment letter outlining the mission of the coalition and expectations of coalition members. • Recruit at least 2 representatives per sector for each of the 4 SHIP sectors • Recruit at least 2 subject matter experts for obesity and tobacco • Examine and improve the organizational structure and functioning of the Healthy Communities Collaborative • Measures • Coalition membership will include diversity- geographical, racial, ethnic (Caucasian and Hispanic), subject matter experts. • Members have committed via a letter of support to participate in the coalition. • A charter defining coalition structure, action plan and member roles and responsibilities.
