  9. Process • First Point of Contact for Sales Support- Agency Office • All applications must be submitted through the GA’s office to the local MetLife Sales office. This will ensure the proper identifying forms are attached. • All applications require a transmittal form (identifies the case as AIMCOR) • All policies will be mailed to the GA’s office from your Met Sales office. GA’s office will be responsible for forwarding to Agent/Broker • DI Sales Specialist and Rep • Your main point of contact at MetLife • Will receive all new applications and ensure they are in good order • SS can also provide assistance with Appointments, Product questions, Competition questions, Commissions questions • Applications can also be found at: https://eforms.metlife.com/wcm8/

If institutionalized or hospitalized, limitation will not apply to that period.
