  2. MDHI HPRP PROGRAM • $5 million ARRA Funding from DOH targeted to MDHI Continuum of Care • $3.2 Million additional ARRA Funding through City and County of Denver • October 1, 2009 to July 16, 2012 • Leveraged additional $4 million of TANF Emergency Contingency Funds

  3. MDHI HPRP PROGRAM • Funding for: • Homelessness Prevention • Rapid Re-Housing • Rental Assistance • Eviction Assistance • HMIS Expansion

  4. MDHI HPRP PROGRAM • Coordinated Intake and Assessment • County Lead Agencies • Collaboration Established with 27 Agencies in Metro Denver

  6. OUTCOMES • Program served 2,097 households • 1,020 Single Households • 1,080 Families • Program served total of 4,809 persons • 3,241 through Homelessness Prevention • 1,596 through Rapid Re-Housing

  7. OUTCOMES • 2,765 persons prevented from becoming homeless • 1,047 persons helped to obtain housing through Rapid Re-Housing
