Clinical Practice Journey • Older adults and rehabilitation for most of my career. • Worked in clinical nurse manager and clinical nurse specialist roles for majority of the last 30 years. • Moved to NZ in Dec 2006. • Waikato DHB supported through Masters programme with prescribing papers.
The Academic Journey • RGN – Guys Hospital, London – 1985 • Dip Nursing, London – 1993 • BSc (Hons), Manchester – 2002 • PG Cert (Clinical Rehab ) Otago – 2008 • Masters Clinical Nursing, Auckland -2012.
Then what? • No role for NP older adult in DHB supported by Older Persons Services. • CNS roles disestablished as part of DHB wide review. • Back to the stroke ward on rostered shifts.
Visible and Valued • Applied to NZNO for funding to attend conference Aug 2012. Swapped rosters to be able to attend on my days off. • Had a phone call from previous manager who let me know of a role with HCNZ. • Moved to the private sector, new perspective. • Overworked, new role, no idea of what was required. • BUT -supportive managers, have recruited more staff to keep me!
Following the NP dream • Made contact with 2 NP’s to provide me with mentorship – Denise Thatcher and Rachel. • Had verbal support from HCNZ for NP role. • Hurdles • – prescribing support and practice • - demonstrating advanced nursing practice • Solution • Accepted as DP for ACC nursing contract • Residential Eldercare Services Ltd
RES Ltd • Virtual GP practice, providing primary care to residential care facilities within Waikato. • Currently 6 facilities and due to expand further later in the year. • Uniquely offers GP and NP services • On-call 7 days a week 8am -8pm.
My experience highlights as an NP intern • First day – fake it til you make it! • Providing regular support to the facility teams, and challenge to empower nursing staff. • Cultural differences in each facility, need a different approach in each one. • Being able to practice as advanced clinician, assessing patients, ordering investigations, commencing treatment within standing orders, recommending acute hospital admission, maintaining health and wellness with health screening.
The future • Aim to get portfolio to Nursing Council within next 2 months. • Next NP Intern to be found • Explore NP role within HCNZ
