Data Collection Tools-Changes • Pre ART Register • Unique number has MFL prefix e.g. 11740-00001 • Extra columns for TB status data added • Columns on prior pregnacies and their outcomes including HIV exposed infant numbers if any • Quartely follow-up columns on • CD4/WHO staging, • CTX/TB status • Patient status i.e. Lost, dead, etc • ART Monthly Register • Unique number has MFL prefix • Extra columns for pregnancy (ies) while on ART • Columns (ab) to (bk) from months 25th-48th month.
