Case Management • Identify those traits that are needed by a professional nurse case manager: • Communication, independence, flexibility, creativity. • Case Managers are typically NOT direct care providers, i.e., they do not perform advanced clinical nursing skills or techniques on clients. • Identify patients / clients in need of nursing case management services • Typically, these patients are in need of a case manager to coordinate care across a broad spectrum of services, e.g., skilled nursing care, placement in facilities, durable medical equipment, pharmaceutical services, physical & occupational rehabilitation. Typically not acute cases, or those not requiring extensive follow-up. • Note that nurses serving other functions or having other job titles , e.g., public health nurse or school nurse or occupational health nurse may also be performing “case management” and be working as a “case manager” if they are caring for individuals with complex health problems.
