  2. ACCESSING OTHER SERVICES THROUGH COORDINATION

Reasons for Coordination • Provides expanded program services to meet the needs of Tribal Elders • Improves quality of life for Tribal Elders • Increases awareness and expands community support for the Tribal Elders Program • Supports family efforts to care for loved ones at home and in their community • Increases your knowledge about the programs of Title III

Reasons for Coordination (Continued) • Improves staff effectiveness • Reduces duplication of services • Develops partnership to address unmet needs • Informs community about services available to benefit Tribal Elders • Expands resources including funding, staff training, facilities, equipment, etc. • More money in some cases

  5. Ingredients for Success • Coordination – open discussion, active listening, sharing data and information • Collaboration – Memorandum of Understanding; having a voice in how/when/where services are provided; joint funding commitment • Cooperation – implement the coordinated service project; provide follow-up; agree on continuation plan • Partnership – A contract entered into by two or more persons in which each agrees to furnish something

  6. TITLE III/VI COORDINATION IS NOT A CHOICE ---IT’S THE LAW

Older Americans Act Requirements • Sec 614(a)(11) – A Tribal Organization receiving Title VI funding must assure it will coordinate Title VI services with services provided under Title III in the same geographical area • Sec 306(a)(11)(B) – Each Area Agency Plan shall provide information concerning services to older Indians and assure the Area Agency will coordinate services with those provided under Title VI • Sec 307(a)(21)(A) – The State Plan shall assure that the State Agency will coordinate programs under Title III with programs under Title VI

Title III/VI Coordination Strategies • Obtain commitment from Tribal leadership to coordinate services with State and Area Agency • Ensure Tribal representation is included on planning work groups at the State and Area levels • Volunteer to host planning meetings • Become a member of an advisory committee at the State and Area levels

  9. Title III/VI Coordination Strategies (Continued) • Share needs assessment data • Encourage open communication • Host public hearings on State and Area Plans • Have a positive attitude • Keep your expectations for success HIGH

  10. Outcomes from Coordination • Menu planning • Transportation • Training • Outreach • Liaison person/contact person • Cooperative buying • Legal assistance • Etc. – add to this list to meet your program’s needs

  11. Coordination with other Resources • Tribal programs (IHS, BIA, Job Training, Housing) • Other federal agencies (SSA, CMS, NIH, HUD) • Other state/local agencies (Disaster Services, Health) • Other groups (AARP, VFW, Churches, Red Cross) • Etc. – add to this list to meet your program’s needs

Outcomes from Coordination with Other Agencies • SSA outreach to elders • CMS enrolling Elders for Medicare Part D • CDC National HIV Testing Mobilization Campaign • The American Diabetes Association (ADA) Diabetes Expo • American Red Cross education classes for senior citizen on disaster • CDC What You Can Do to Prevent Falls • SSA low income subsidy (LIS) • Etc. – add to this list to meet your program’s needs

  13. Summary Your Tribal organization assured coordination with Title III when it applied for Title VI funding. HAVE A PLAN TO ACCOMPLISH THIS STATUTORY REQUIREMENT.

  14. Summary(Continued) Title III has access to resources and information that Title VI programs are still struggling to achieve. ACCESSING STAFF TRAINING AND TECHNICAL ASSISTANCE CAN HELP IMPROVE CAPACITY TO SERVE YOUR ELDERS.
