Patient Safety Heroes The K-HEN Patient Safety Heroes are individuals (or teams of individuals) who have performed at a high level of accountability and have been instrumental in one or more of the following areas: • Uncovered new methods of QI and safety • Trained staff members on the importance of patient safety • Prevented harm for a patient at risk • Other improvement activities that deserve recognition.

Patient Safety Leaders KHEN Patient Safety Leaders are statewide leaders who have received additional education in patient safety culture and teamwork concepts. Patient Safety Leaders may be asked to provide consultation and technical assistance to KHEN hospitals as they identify barriers and work through quality improvement strategies.
