  2. State Primary Care Grants Program Eligibility: • Public and Non-Profit entities • Provision of Primary Care services • Must serve medically underserved population (low income, uninsured, disadvantaged minorities) • Cannot replace “government” insurance (Mcaid, Mcare, etc.)

  3. State Primary Care Grants Program Funding: • Current Appropriation = $1.6 million • $6.4 million spent since FY 2005

  4. State Primary Care Grants Program Awards: • Current Awards = 56 • 28 Urban and 28 Rural • Cumulative Awards (since FY ’05) • 87 Urban and 105 Rural

  5. State Primary Care Grants Program Individuals Served: • Current Service = 25,030 • 10,600 Urban and 14,430 Rural • Cumulative Service = 72,822 • 34,716 Urban and 38,106 Rural
