Asks for information about the driver and the incident or pattern of behavior that justifies a driver reexamination
Must include name, address, telephone number and signature
Confidential to extent permitted by law
13. 13 Physical and Mental Standards for Drivers257.309 of the Michigan Compiled Laws When a driver experiences an EPISODE, the driver’s license is either denied or indefinitely suspended until a medical statement shows either of the following:
The condition is under control and all symptoms or conditions that affect driving ability are corrected, cured, or controlled or abated for not less than 6 months (12 months for chauffer/CDL), or
Episode free for previous 6 months (12 months for chauffer/CDL)
The 6- or 12-month period may be reduced or eliminated upon Department review PHYSICIAN’S STATEMENT – ONE TIME EVENT – SUPERVISED PRESCRIPTION PHYSICIAN’S STATEMENT – ONE TIME EVENT – SUPERVISED PRESCRIPTION
14. 14 “Episode” means any of the following: A condition which causes or contributes to any of the following: lapse of consciousness, blackout, seizure, fainting spell, syncope, other impairment of the level of consciousness.
A condition which causes or contributes to an impairment of an individual’s driving judgment or reaction time or affects an individual’s ability to safely operate a motor vehicle.
