Entrance exam (pre-enlistment, pre-commission)No requirement for AD to have periodic screening medical examination; annual dental exam requiredProviders expected to address medical qualification, preventive services during all visitsAnnual Preventive Health Assessment (PHA)Retirement/separation examination if requested or involuntary separation.

    3. 3 Annual Preventive Health Assessment (PHA) Records review, at minimum, to: Identify any changes in health status Arrange evaluation, if needed Ensure completion of military required laboratory tests (HIV, G6PD, DNA, etc.) Targeted interventions based on: Age, gender, occupation, behaviors, other risk factors Recommended clinical preventive services (per USPSTF)

    4. 4 PHA continued Blood pressure screening at annual dental exam BMI calculated; smoking and physical activity assessed at annual fitness test Track requirements in automated system
