RIGHT SIDE of Folder • HPRP Assessment • Release of information (ROI) • HMIS • Vendors • Other Service providers (Legal Services, Credit help, budgeting, etc.) • Verification of Income or Self-declaration of Income(when applicable) • Self-declaration of Housing Status (when applicable) • Rent Reasonableness (for rapid re-housing only) • Housing Inspection (for rapid re-housing only) including lead paint test for housing built before 1978 and having children under the age of six) • At three months—repeat assessment • Maintain contact with client each month to determine continued need • File closure form
LEFT SIDE of Folder • Purchase Order • Verification documentation (Lease agreement, letter from landlord, utility costs [deposits, estimated monthly charge] etc.)
