How does Clinlert work? Web Interface • Proof-of-concept web-based application that sends specific, customizable alerts directly to a clinician’s pager or e-mail • Alerts can include: • Therapeutic drug monitoring levels, such as vancomycin • Positive microbiology culture results • Any lab available in VistA, such as urine antigen results or serum chemistries • Allows clinicians to be informed of clinically relevant events to increase data accessibility, clinician efficiency, and convenience • Initial Setup • Clinicians login to Clinlert using their VistA credentials • Configure patient lists, alert criteria, and other settings • Alert System • Once configured, clinicians only need to login to Clinlert if they want to change their settings • During a clinician’s contact hours, Clinlert will continually monitor his or her patients and send alerts as appropriate Utility and Applicability 1. • Every 10 minutes, Clinlert polls the VistA system • All new patient data is retrieved and analyzed • If a patient’s data meets an alert criteria, the appropriate practitioner will receive a text page or e-mail VistA Servers • Increases data accessibility • Relevant data reaches healthcare providers quicker • Once available in VistA, new data arrives to clinician’s pager in less than 10 minutes via a continuous monitoring system • Increases clinician efficiency • Clinicians do not have to continually check patient charts or refresh patient profiles in anticipation of a new lab or microbiology culture result • Clinicians can work on more important tasks until new patient data is available 2. Clinlert (MDWS) 3. -ALERT- Similar & Future Applications Practitioner • The use of a rule system causing a clinician-oriented electronic page can be used in a variety of settings • Automated hospital sepsis surveillance system • More advanced drug interaction rule systems, such as drug/lab interactions (e.g. acute renal failure and metformin) • Paging when certain orders are placed (e.g. vancomycin) • Notification when patients from a certain outpatient clinic are admitted to the hospital (e.g. post-transplant or HIV) • MDWS • “Medical Domain Web Services” • Similar technology used in VistaWeb • Supported by the Ann Arbor VA • By using MDWS instead of MUMPS code, Clinlert is more scalable and requires less site configuration VistA MDWS Web Service (Ann Arbor VAMC) Clinlert (Microsoft ASP.NET)
