A Simple Two-Step Process for Taxpayers Contribute to GOAL: Send the following items to Harvester Christian Academy Tax Program: (All forms can be downloaded from the website or requested from the Advancement Office) http://harvesteracademy.com/giving/tuition-tax-credit/tax-credit-forms/ – Complete the GOAL Scholarship Contribution Form – Sign page 2 of the IT-QEE-TP1 Form – Make your check payable to Georgia GOAL Scholarship Program. Credit card payments may also be made through a Credit Card Authorization Form found on the website. Harvester school officials and GOAL Scholarship Program officials will manage the paperwork and approval process from this point. Claim the Credit: – File your Georgia income tax return and take the credit (GOAL will send tax filing instructions early in 2011 to assist you)
