  6. Medical Services • Mrs. Rene Madigosky, R.N., M.S.N. (Ext. 5102) • Mrs. Anna Marie Benlian, R.N., A.A.S. (Ext. 5470) • State mandated physical due by September 30th • Medication • All medication must be delivered to the nurse in the original prescription bottle or over the counter medication packaging. • All medication must be delivered to the nurse by an parent/guardian or arranged with the nurse for a student to deliver. • No student may carry medication on their person • One exception - may carry inhaler w/doctor’s note • Physician’s notes and medical issues should be submitted/communicated directly to Mrs. Madigosky

  8. Purpose of Camping trip • To offer students opportunities to enrich their personal growth • To transplant our 6th grade curriculum to the outdoor environment for 3 days

Important Information • Dates of trip: May 27th, 28th, and 29th • Camp Canadensis in Canadensis, PA • Approximately 2.5 hours away • Check out their website: www.canadensis.org • Check the HMS website (6th grade page) for all forms and updated information as it becomes available • Camp Canadensis information letter with several enclosures mailed late October.

  10. Medical Information • Medical facility staffed 24 hours a day with currently licensed nurses and physicians nurses and MD (provided we have a volunteer) • Medication distribution is confidential under guidance of medical professionals • Accommodations for food allergies • Peanut free facility

  11. Cost • $160 per student, no cost increase for 2015 • Includes bus, room/board, all activities • Check or money order only • Installments due by November 15thand February 9th

  12. Parent Chaperones • All applicants must fill out application in full First preference given to those in the medical field • All other parents chosen through a lottery system • Once selected, all chaperones must apply and pass two State of Pennsylvania background clearances (criminal and child abuse) • Chaperone cost, $110

  13. Please Note… • Students must commit to attend camp by April 3, 2015 • Discipline issues may prevent a student from attending the trip • Check your family schedules now to avoid conflicts with the camping trip on May 27th, 28th, and 29th

  14. Cabins • Student choice of 3 friends to bunk with • 9-12 students total in cabin • 2-3 parent chaperones in cabin (dads with males, moms with females) • Electricity and running water in all cabins

  15. Activities • Randomly selected groups of 21-24 students and 4-6 parent chaperones • Students are encouraged to try all activities • Examples include: boating, cooperative games, orienteering, swimming, t-shirt art, climbing “the wall,” cooking, pond study, floor hockey, bracelet art

  16. Meals • Many selections for breakfast, lunch and dinner • Sensitive to all food allergies/ peanut-free facility

  17. Band & Orchestra Parents of band students should report to room 143 after 7th period Parents of orchestra students should report to room 142 after 7th period
