  2. Since its launch at the end of 2005, the Montgomery Cares Program, a Public Private Partnership, provides primary health care to low income, uninsured adults living in Montgomery County.

  3. The Need in Montgomery County • The U.S. Census Bureau reported that in 2008, approximately 14% (nearly 100,000) of adult County residents lacked adequate health insurance.

  8. Montgomery Cares Eligibility criteria • 18 years or older • Residing in Montgomery County • No other medical insurance • At or below the 250% Federal Poverty Level

  9. ProyectoSalud Clinic • Founded as a 501 (c) 3 Corporation in October 1997 • Clinic open its doors in April 1998 in two half-day sessions per week, serving 140 patients the first year • Currently the clinic serves more than 5,000 patients in its two clinic sites in Wheaton and Olney

  10. Population Served (1) • Latino/Hispanic Wheaton clinic 90% Olney clinic 65 • Gender Female 73% - Male 27% in Wheaton Female 67% - Male 33% in Olney • Close to 60% of patients immigrated from Central America (Salvadorians the highest proportion)

  12. Population Served (3) Morbidity • Mainly Chronic Diseases • Diabetes • Cardiovascular Diseases – High Blood Pressure

  13. Services Provided(1) • Primary Adult Healthcare • Internal Medicine • Gynecology Services • Behavioral/Mental Health • Diabetes Education and Management • Laboratory Services and Medications • Referral to Specialty and Support Services

  14. Services Provided(2) Approximately a quarter of patients need at some point a specialty referral every year. Places for referral include: • Catholic Social Network • Project Access – pro-bono services • Local Hospitals – Suburban, Holy Cross, Montgomery Genera, other. • County Agencies – service eligibility units

  15. Funding Sources(1) Patients Fees • Initial and regular visits $0-$60 • Sliding scale according to income level and number of family members • No patient is turned away for inability to pay • Affordable lab work (via Quest), EKG, and pap smear

Funding Sources(2) MedStar Montgomery and Suburban Hospitals • Formal partnerships (fifth year) • A representative from each hospital is a member of Proyeto’s Board • Subsidy to run new clinic site in Olney • Subsidy to run extended hours in Wheaton • Support w/cancer screenings and health education, and specialty referrals.

Challenges and Opportunities • Implementation of new electronic health record • Becoming a Medicaid provider • Need for additional funds to strengthen health education & quality improvement • Long waiting time for specialty services outside clinic • Current limited physical capacity to expand • Participating in a Patient-Centered Medical Home (PCMH) project
